Connecting Home is a service for the Stolen Generations working across South East Australia. Connecting Home works using different programs to support survivors on their journey of healing through walking with individuals and families on this journey and using goal setting principles.

In May 2017 Connecting Home became registered to provide NDIS services to the Stolen Generations and their families. Connecting Home provides the following services as part of the NDIS;

Support Coordination
Plan Management
Direct Support Services
At Connecting Home we believe that everyone is unique and will require tailored supports and programs to assist with an individual’s journey of healing.

Support Coordination helping you achieve your goals. These goals will be in your NDIS plan completed with your planner and our Support Coordinators will help you action your plan.

We will help you choose a culturally safe provider and assist you in finding services that match your needs.

Plan Management and Direct Support Services are supported through Connecting Home’s Support Coordination Program.
These services are available so as to be able to provide options to the Stolen Generations and their families that are Culturally Safe and appropriate and enable Connecting Home to also extend support to individuals and families through our Support (Case Management) Program if needed. The services all ensure providing holistic support to Individuals and families

We will assist with your connection to community, as well as Aboriginal and Mainstream agencies.

As Connecting Home’s services cover South Eastern Australia, we can assist you within your own community.
